Why health care still asks for fax
Many US practices, hospitals, pharmacies and insurers still exchange paperwork by fax, because their systems don't always connect and fax is a familiar, accepted channel. What people commonly fax
- Medical records requests and authorizations – for example a signed form allowing one practice to send your records to another.
- New-patient and intake forms before a first appointment.
- Insurance claim forms with receipts or itemized bills.
- Appeals of a denied claim, with the denial letter and supporting documents.
- Work-related forms such as FMLA certifications or short-term disability paperwork that a doctor completes or an employer requests.
- Pharmacy paperwork where the pharmacy asks for a fax.
Where to find the right fax number
Health care fax numbers are often department-specific. A wrong number can delay things for weeks.
- Ask the office directly and repeat the number back. Ask which department it reaches (records, billing, referrals).
- Look on the form. Claim and appeal forms often print the fax number for that exact process.
- Check your insurer's letter or member portal. Appeals in particular usually have their own fax number on the denial letter.
- Medicare: there is no general Medicare fax number. Appeals of Original Medicare claims go to the contractor listed on your Medicare Summary Notice. A few central Medicare units are listed on our Medicare (CMS) directory page.

- Identify every page. Write your full name, date of birth and member, claim or reference number at the top of each page. Faxed pages get separated.
